Quick self-assessment for IT & business leaders

For each statement, rate your organization on a 0–3 scale:

  • 0 = Not at all true
  • 1 = Somewhat true (ad-hoc, inconsistent)
  • 2 = Mostly true (formalized, but with gaps)
  • 3 = Fully true (consistent, mature practice)

Instructions

For each layer, review the guiding questions.

Record strengths and weaknesses (qualitative view).

Score each gap using the Gap Severity Index (0–3):

  • 0 = No Gap (fully mature practice)
  • 1 = Minor Gap (inconsistent, low risk)
  • 2 = Moderate Gap (systemic, some business risk)
  • 3 = Major Gap (critical risk, must fix urgently)


Assign a priority (High / Medium / Low) to guide action planning.
